The Primary Care Specialist will have a discussion with you regarding your client’s requirements and the services we can potentially offer.
If you wish to proceed, you’re Primary Care Specialist will arrange a meeting to undertake a risk assessment.
A fully costed care package proposal will be forwarded to you.
When you are ready and wish to proceed, we will finalise the care plan with you and your client.
We will identify suitable nurses or care assistants by evaluating education experience, and training requirements. We will arrange for them to meet you and your client.
When you are happy with the staff, we will arrange for specific training to be implemented if complex clinical/behavioural issues have been identified.
If the delegated tasks have been identified, our clinical team will assess staff to ensure competences have been met.
Care package commences.
Ongoing supervision and delegation of task management if required.
Continual reviews of service maintained with service user, family and case manager.
